FORM #1: CAS Activity Proposal

You can use this online form to submit your proposals for activities for the year.

CAS Student Handbook SY 2010-2011

This is the complete handbook. Save a personal copy but please refrain from printing the whole document (unless you REALLY have a strong reason to do so). We're working on making the forms available online to further reduce the need to print and photocopy. Thank you for helping us promote and model responsible use of paper.

CAS Self-Review forms

Use the questions in this document to develop a personal inventory of skills, talents, achievements and other helpful information about yourself. Your responses and reflections will certainly be helpful when you plan and execute your projects/activities.

CAS Planning form

Use this form to plan out the start of your year. Remember that your actual plan will evolve as the year progresses, so don't sweat it. The important thing is that you get off to a good start. (Juniors, this is what you need for your first interview with the CAS coordinator.)

CAS Program Review form (Seniors, take note!)

This is an evolved version of the Planning form (see above) to help summarize what has been done so far and where focus should be for future planning.


Check out this list of possible CAS opportunities and ideas. Some may be particular to the Baguio area, but the general ideas are universal and can certainly be adapted to our setting here in Subic.
